You'll basically be tested on things such as your abilities reactions under certain situations, like those that create strong emotions or stress and also there is a dueling test. During these your ability will be monitored for the strength and then they'll be able to create a limiter specifically for you.
They also like to re-test once a year at the least, just to make sure there aren't any changes to how strong the psychic ability is and if there is, they can make adjustments to the limiter as needed.

... Have there ever been cases where an ability might be too strong to be limited, however?

Well, usually, for most clients, the limiters work fine and there is never any problem. A staff member of mine has mentioned a client of hers is a bit of an oddity with limiters not working, but I think that's more of an individual case.
All we can do is get you fitted with a limiter, see if it works and if you do experience any issues, you're free to come back and we'll re-look at everything.
